Person-centred support for children, young people and their families

E2E Homecare provides individually planned care and support for children and young people, helping them remain safe, participate in family and community life and achieve outcomes appropriate to their individual needs.

We recognise that supporting a child or young person means working closely not only with them, but also with their parents, carers and the professionals involved in their care and development.

Individual Support

Depending on the child’s or young person’s assessed needs and the services E2E is commissioned and registered to provide, support may include:

  • Personal care
  • Daily routines
  • Support with independence and life skills
  • Community access
  • Social and recreational activities
  • Support attending appointments
  • Education-related routines and activities
  • Respite support for families
  • Medication support where appropriate
  • Support with physical disabilities
  • Support with additional needs
  • Emotional and behavioural support within the agreed care plan

Supporting Families

We understand the importance of consistency and trust when a family welcomes care workers into their home.

We aim to build a consistent team around the child or young person, taking time to understand their communication, personality, preferences, routines and what helps them feel safe and comfortable.

We work collaboratively with parents and carers, social workers, schools, commissioners and relevant health and therapy professionals to support coordinated care.
