Supporting people to live fulfilling lives at home and within their communities.
At E2E Homecare, we believe good care should enable people to do more than remain safe at home. It should support people to maintain their independence, relationships, interests, routines and connection with their community.
Our Community Access service is individually designed around what matters to each person. Whether someone requires occasional support to get out and about or community access forms part of a larger homecare, complex care, 24-hour or 2:1 care package, our team provides the support needed to participate safely and confidently in everyday life.

Community Access for People with Complex Needs
For people living with spinal cord injuries, acquired brain injuries, neurological conditions, physical disabilities or other complex health needs, accessing the community may require additional planning and specialist support.
Where assessed as necessary, E2E Homecare can provide 2:1 staffing to support safe transfers, specialist moving and handling, mobility, positioning and other individual requirements.
Our care teams can also support people to attend hydrotherapy, physiotherapy, rehabilitation programmes and other therapeutic activities, working alongside the professionals involved in their care.
Any relevant clinical needs, risks, equipment and emergency arrangements are considered as part of the person’s care planning and risk assessment.
More Than Getting From A to B
Community access isn’t simply about accompanying someone outside their home.
It is about helping people retain choice, control, confidence and meaningful participation in everyday life.
We take time to understand what each person wants to achieve and build their support around those goals — whether that’s returning to a favourite activity, seeing friends, attending rehabilitation, going shopping, returning to work or simply enjoying greater freedom.
Part of Your Individual Care Package
Community Access can be provided as a standalone support service or incorporated into an individual’s:
For complex packages, we work closely with families, case managers, commissioners, rehabilitation professionals and multidisciplinary healthcare teams to ensure community activities form part of a coordinated and person-centred approach.
Your life. Your choices. Your community. Our support.
